About Us

Crystal Oaks Assisted Living and Skilled Nursing were established by Jefferson Memorial Hospital in 1995 and 1997, with the mission of expanding their high quality healthcare services to the local community.

In 2007, the hospital created Crystal Oaks Long Term Care, and transferred their assisted living and skilled nursing operations to this independent corporation. Crystal Oaks purchased the existing complex from the hospital in 2008, and expanded their services to include short term rehabilitation and respite care at The Oaks Rehabilitation Center.

Crystal Oaks is the only independent not for profit care community in Jefferson County.
