Crystal Oaks Skilled Nursing, Assisted Living and The Oaks Rehabilitation Center are dedicated to providing excellent health care services to our community. We constantly strive to enrich our resident’s lives with high quality services and superb customer service. Our well-trained, dedicated staff will be here to help you and your loved one with whatever your needs may be: Long Term Care, Assisted Living, Memory Care, Hospice, Respite Care or Short Term Rehabilitation at The Oaks. Our staff is ready to assist you. We take great pride in our facility and staff and we invite you to visit us at any time. If you need services or have questions we are available to assist you.

Jim has been working in healthcare all of his adult life. From 1978 to 1989 he worked for St. Anthony’s Medical Center. In 1989 he moved to long term care, taking a position as Assistant Administrator at Truman Restorative Center, a nonprofit 220 bed skilled nursing facility in St. Louis. He received his Nursing Home Administrators license in 1992, and was certified by the American College of Healthcare Administrators in 1995. Jim took over as Administrator of Truman Center in 1997 and then Chief Executive Officer in 2000. In 2005 Jim took over as Executive Director of Crystal Oaks for Jefferson Regional Medical Center. Under his direction, Crystal Oaks successfully spun off from JRMC in 2007 to an independent not for profit entity. The new corporation, Crystal Oaks Long Term Care, purchased the facility from JRMC in 2008. Financing was secured in 2010 for the addition of a 25,000 square foot rehabilitation center adding 32 beds to the Crystal Oaks complex. Today, Crystal Oaks maintains its excellent reputation in the community as a premier long term care and short term rehabilitation provider.

Cyndi Morgan

Cyndi has worked in the healthcare field for over 20 years. She obtained a Bachelor’s of Social Work degree in 1991 and has worked as a medical social worker with hospice, in long-term care and in several acute care hospitals. During her employment with Jefferson Regional Medical Center, Cyndi decided to further her education and pursue a career within management. In 2008, Cyndi completed a Master’s of Business Administration/Health Care Management degree and shortly thereafter was promoted to Social Work Supervisor, then later to Director of Social Services. In 2011, Cyndi returned to long-term care as the Director of Social Services with Crystal Oaks. In early 2013, Cyndi was promoted into the Assistant Administrator position. She obtained licensure with the Missouri Board of Nursing Home Administrators in 2014. Cyndi has a strong interest in customer service, quality assurance and fundraising. She is delighted to work for a not-for-profit organization that focuses on providing quality care to seniors.

Michelle Rolfi

Michelle started her career in healthcare when she was a teenager. She has worked as a caregiver primarily with the geriatric population, but also with special needs children and mental health patients. During her many years of working with the geriatric population, Michelle developed a strong interest and passion for helping those diagnosed with dementia.

In March of 1998, Michelle joined the Crystal Oaks team. She has worked with activities, social services, as a dementia care coordinator and most recently as the Assisted Living Manager. In late 2013, Michelle was promoted to Assistant Administrator.

Michelle has been an active volunteer with the Alzheimer’s Association for many years. She was instrumental in bringing the Alzheimer’s Association’s Annual Memory Walk to the Crystal Oaks campus. Additionally, she facilitates the Alzheimer’s and Related Dementia Community Support Group that is held monthly at the Crystal Oaks campus. Due to her experience and expertise with dementia, Michelle provides dementia training for our staff. Also, Michelle is the host for our support dog, Lolly. Michelle and Lolly, a beautiful golden retriever, attended the required support dog training and now Lolly is part of the Crystal Oaks team. Michelle and Lolly come to work with the same goal: to brighten the lives of our residents.

Tammy Cashion

Tammy has pursued and maintained a career in the health care profession for over 25 years. She started her career as a Certified Nurse Assistant in 1981. After spending several years in long term care, Tammy continued her education and obtained her Practical Nurse license and then her Registered Nurse licensure. During her nursing career she gained valuable knowledge and experience from working as a charge nurse, supervisor, and specialized wound care nurse in various types of health care facilities.

Her dedication and love for the geriatric population prompted and motivated her to pursue a management position in long term care. She has since established a position as Director of Nursing for the past 8 years. Tammy’s goal as Director of Nursing has not only been to improve the quality of care delivered in long term care facilities but to improve the quality of life in the geriatric patients who seek rehabilitation.
